When a child lives in a long-term care facility, it is their home. With an average length of stay of more than three years, our health care facilities need to be more than a hospital or subacute rehabilitation facility. They need to be warm and familiar homes to each and every child. So while our buildings meet the highest standards for health care facilities, they are also homes where our children will spend a significant portion of their young lives.

Both of our clinical care facilities are recognized as Five-Star facilities by the Centers for Medicare and Medicaid, the highest level of recognition awarded by CMS. This recognition signifies our exceptional performance in State health inspections, high clinical staffing levels, and consistent quality of care indicators.

Our facilities are located in the South Bay cities of Campbell, San Jose, and Saratoga in Santa Clara County, and are conveniently located minutes from world-class hospitals including the Lucille Packard Children’s Hospital, Santa Clara Valley Medical Center, and Kaiser Permanente Medical Center. As Northern California’s only providers of pediatric subacute care, we also work with referring hospitals throughout Northern California to place children in need of our unique services.

A Holistic Approach to Pediatric Services

Children’s Health of Northern California (CHoNC) is California’s comprehensive solution for long-term rehabilitation and comprehensive medical care for machine-dependent children. Our high-acuity nursing care is delivered under the direction of experienced pediatric medical directors in a warm and inviting home-like environment, while our pediatric day health, respite, and education programs are designed with the inquisitive child and busy family in mind.

School Services

CHoNC collaborates with the Santa Clara County Office of Education to provide educational services to all patients residing at both CHoNC Pediatric Hospital and Saratoga Pediatric Subacute.

Services include:

  • Onsite school
  • Offsite school (when appropriate)
    • Individual aides or nurses provided
    • Busing to appropriate schools
  • Bedside school when needed
  • Age & ability appropriate curriculum
  • Assistive technologies
  • Sensory stimulation
